Our Client integrates with physician practices to provide non-invasive physical diagnostic and advanced blood tests to identify the degree of heart disease, vascular disease, and stroke risk in their patient population. Our iCoach team is responsible for the successful implementation of the Healthy Heart Program with each patient. Through the Heart Healthy program, the iCoach: Guides new patients into the program by explaining testing process and Healthy Heart Program Promotes the need for testing to patients based on physician recommendation Provides the patient with unlimited one-on-one coaching Gives the patient tools for personal success Tracks patient progress through an interactive website Reports progress to referring physician.
